Job summary

Creative Solutions in Healthcare, Inc. seeks a dedicated Occupational Therapist (OT) to join our in-house team and enhance residents' independence and quality of life through targeted ADL/IADL interventions.

You will work within a multidisciplinary setting to implement personalized plans and ensure safe, effective care in a SNF/long-term care environment. The role requires a current Texas OT license, good standing with regulatory boards, and familiarity with Net Health EDS.

Qualifications

• Active and valid Texas Occupational Therapy license
• In good standing with regulatory agencies and licensing boards
• SNF or long-term care experience preferred
• Current knowledge of occupational therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life
• Assess and interpret patient evaluations and test results
• Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in ADLs and IADLs
• Assess patients’ home and living environments and recommend modifications based on their needs and capabilities
• Recommend adaptive equipment and provide training to support independence and safety
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical education and support to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab
