Job summary

Creative Solutions in Healthcare, Inc. seeks a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis, delivering person-centered care to residents in a skilled nursing setting.

You will assess function, develop individualized ADL/IADL plans, modify environments, train on adaptive equipment, and collaborate with an interdisciplinary team to improve safety and independence. Texas OT license required; new graduates welcome.

Qualifications

• Active Texas OT license.
• In good standing with regulatory agencies.
• SNF or long-term care experience preferred.
• Familiarity with Net Health Electronic Documentation System.
• Knowledge of occupational therapy treatment practices.

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans using a variety of therapeutic techniques to maximize ADLs and IADLs.
• Assess patients’ home and living environments and recommend modifications based on their needs and capabilities.
• Recommend adaptive equipment and provide training to support independence and safety.
• Ensure all treatments are delivered in accordance with the established plan of care.
• Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team.
• Provide clinical education and support to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations.
• Comply with all applicable rules, regulations, and procedures.
• Attend required meetings as designated by the Director of Rehab.
