Job summary

Eastern Star Home and Campus in Oriskany, NY is seeking an Occupational Therapist (OT) for a PRN opportunity. You will evaluate residents and develop plans of care to help them perform daily activities safely and independently.

Join a collaborative rehabilitation team and provide ADL-focused therapy, monitor progress, and adapt plans. Prior SNF or rehab experience is preferred, and new graduates are welcome to apply.

Qualifications

• Active New York OT license required.
• Graduate of an accredited Occupational Therapy program.
• Strong clinical, communication, and teamwork skills.
• SNF or rehabilitation experience preferred but not required.

Responsibilities

• Evaluate residents and develop individualized OT plans of care.
• Provide skilled treatment focused on ADLs, strength, balance, and independence.
• Establish treatment goals and monitor resident progress.
• Recommend adaptive equipment and safety strategies for daily function.
• Adjust treatment plans based on resident needs and progress.
• Maintain accurate and timely clinical documentation.
• Collaborate with COTAs, nursing staff and rehab professionals.
