Job summary

Post Rehab is seeking an Occupational Therapist (OT) to join the rehabilitation team at Eastern Star Home and Campus in Oriskany, NY. This full-time role offers individualized therapy to help residents improve daily function, build confidence, and maintain greater independence.

You will perform OT evaluations, create plans of care, address activities of daily living, strength, balance, cognition, and will collaborate with COTAs, PT, SLP, nursing, and other team members to ensure high-quality

Qualifications

• Active New York OT license required.
• Graduate of an accredited Occupational Therapy program.
• Strong clinical, communication, and organizational skills.
• New graduate Occupational Therapists are welcome to apply.
• Skilled nursing or rehabilitation experience preferred, but not required.

Responsibilities

• Complete occupational therapy evaluations and develop individualized plans of care.
• Provide skilled treatment addressing activities of daily living, strength, coordination, balance, cognition, and functional performance.
• Establish appropriate therapy goals and monitor resident progress.
• Introduce adaptive techniques and equipment to support safety and independence.
• Adjust treatment plans based on resident needs and response to therapy.
• Provide education to residents and caregivers to support continued progress.
• Maintain accurate and timely clinical documentation.
• Work closely with COTAs, PT, SLP, nursing, and other members of the interdisciplinary team.
