Job summary

Eastern Star Home and Campus in Oriskany, NY is seeking a Physical Therapist (PT) for a PRN position to support a collaborative rehabilitation team in providing individualized therapy aimed at restoring mobility and independence.

You will perform evaluations, develop plans, guide residents and caregivers, document progress, and coordinate with PTAs and nursing staff. NY license required; new grads welcome; flexible scheduling.

Qualifications

• Active New York Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and organizational skills.
• New graduate Physical Therapists are welcome to apply.
• Skilled nursing or post-acute rehab experience preferred, but not required.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide skilled interventions addressing strength, balance, gait, transfers, endurance, and mobility.
• Establish functional goals and monitor resident progress throughout treatment.
• Modify treatment plans based on clinical needs and response to therapy.
• Educate residents and caregivers on therapeutic exercises and safe mobility strategies.
• Maintain clear and timely clinical documentation.
• Coordinate care with PTAs, nursing staff, and other rehabilitation professionals.
