Oakmont Healthcare in Texas is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will evaluate residents’ needs, develop individualized ADL/IADL-focused treatment plans, and collaborate with the interdisciplinary team to maximize independence and safety.

The ideal candidate holds an active Texas OT license, SNF or long-term care experience, and proficiency with Net Health Electronic Documentation System.

Qualifikationen

• Active Texas Occupational Therapy license required.
• In good standing with regulatory agencies.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.
• Experience with Net Health EHR preferred.
• Ability to manage patients with varying needs.
• Strong communication and collaboration skills.

Aufgaben

• Consult with patients to evaluate their condition and its impact on daily activities.
• Assess and interpret patient evaluations and tests.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess home environments and recommend safety modifications.
• Recommend adaptive equipment and provide training.
• Deliver treatments per the care plan and adjust as needed.
• Monitor patient response and adjust interventions with the team.
• Provide education to patients, families, and caregivers.
• Collaborate with the interdisciplinary team to ensure quality care.
• Complete documentation in a timely, compliant manner.
