Job summary

Creative Solutions in Healthcare, Inc. seeks a compassionate Per Diem Occupational Therapy Assistant to join our in-house rehab team. The OTA will provide person-centered care to residents, plan and administer treatments, and support independence under supervision of an OT.

We value active Texas license, COTA credential, SNF experience preferred, strong communication and teamwork, and flexibility with PRN scheduling. New graduates are welcome; competitive compensation and flexible schedules.

Qualifications

• Active Texas Occupational Therapy Assistant license in good standing.
• COTA credential required.
• SNF or long-term care experience preferred.
• Experience with Net Health Electronic Documentation is a plus.
• Strong communication and teamwork skills are essential.

Responsibilities

• Plan and administer occupational therapy treatments to residents to relieve pain and improve function.
• Provide OT services under the supervision of an OT per physician orders.
• Relay information and referrals to the OT when service requests arise.
• Ensure treatments align with the established plan of care.
• Provide clinical support and instruction to patients, families, and caregivers.
• Monitor patient response to interventions and report changes.
• Complete documentation thoroughly and timely per policy and regulations.
• Attend required Rehab Director meetings.
