Job summary

Creative Solutions In HealthCare is seeking a compassionate Per Diem (PRN) Occupational Therapy Assistant (OTA) to join our in-house rehab team. The OTA will provide person-centered care to residents, under the supervision of an OT, helping restore function and independence.

Qualifications include an active Texas OTA license and COTA credential. Experience in SNF or long-term care is preferred; Net Health EHR experience is a plus. Flexible PRN schedules available.

Qualifications

• Active and valid Texas Occupational Therapy Assistant license.
• Excellent communication and teamwork skills.
• Ability to provide person-centered care and documentation.

Responsibilities

• Plan and administer occupational therapy treatments to residents with disabilities.
• Provide OT services under the supervision of an Occupational Therapist.
• Document thoroughly and timely according to policy and regulations.
• Attend required meetings designated by Rehab Director.
