Job summary

Creative Solutions in Healthcare, Inc. is seeking a Per Diem Occupational Therapy Assistant (OTA) to join our in-house rehab team.

The OTA will plan and administer treatments, provide skilled OT services under supervision, and support patients and families to improve independence and quality of life. The role requires an active Texas OTA license and COTA credential, with SNF or long-term care experience preferred.

Qualifications

• Active and valid Texas Occupational Therapy Assistant license.
• Certified Occupational Therapy Assistant (COTA) credential required.
• In good standing with all regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication and teamwork skills.

Responsibilities

• Plan and administer occupational therapy treatments to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing
• Provide skilled occupational therapy services and interventions in accordance with physician orders under the supervision of an Occupational Therapist
• Respond to requests for service by relaying information and referrals to the Occupational Therapist
• Ensure all treatments are delivered in accordance with the established plan of care
• Provide clinical support and instruction to patients, families, and caregivers
• Monitor patient response to treatment interventions and report changes as appropriate
• Complete required documentation thoroughly and timely in accordance with company policy and state/federal regulations
• Attend required meetings as designated by the Director of Rehab
