Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to join our growing team in a SNF setting. You will evaluate and treat patients, develop individualized plans of care, and collaborate with facility staff to ensure high-quality patient outcomes.

You will also re-evaluate progress, adjust goals, and maintain thorough documentation while supervising Occupational Therapy Assistants. This role emphasizes excellent patient service and education for patients and families.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Treating patients and addressing concerns with clear explanations of injury, progress and outcomes.
• Re-evaluating patient progress, revising plans of care and resetting goals.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
