Job summary

Enhance Rehabilitation currently seeks qualified Occupational Therapist candidates to join our team. You will evaluate and treat patients in a SNF setting, following an individualized plan of care. Provide excellent patient care and service to patients and families.

You will collaborate with facility staff, develop care plans, educate patients, re-evaluate progress, and supervise Occupational Therapy Assistants. Timely documentation is required.

Qualifications

• Qualified Occupational Therapist with SNF experience.
• Develop and implement individualized care plans.
• Document evaluations, progress notes and discharges.

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
