Job summary

Enhance Rehabilitation seeks qualified Occupational Therapist candidates to join our SNF team. You will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure excellent outcomes.

You will develop plans of care, set measurable goals, educate patients and families, and maintain thorough documentation. Supervising Occupational Therapy Assistants is also part of the role.

Qualifications

• Requires current OT license in Illinois
• Experience in SNF or rehab setting preferred
• Ability to educate patients and families on injury, progress and outcomes

Responsibilities

• Evaluate and treat patients following an individualized plan of care
• Develop plans of care and measurable functional goals
• Collaborate with facility staff to ensure high quality care
• Re-evaluate patient progress and adjust plans as needed
• Complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
