Job summary

Enhance Rehabilitation is actively seeking qualified Occupational Therapist candidates to join our growing team in a SNF setting. You will evaluate and treat patients following an individualized plan of care while collaborating with facility staff.

You will conduct evaluations, set measurable goals, document progress, and supervise Occupational Therapy Assistants as part of a dedicated team committed to excellent patient outcomes.

Responsibilities

• Providing excellent care for patients.
• Providing excellent customer service to our patients and family members.
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
