Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our team. In this SNF role, you will evaluate and treat patients following an individualized plan of care and collaborate with facility staff to ensure excellent patient outcomes.

You will oversee OTAs, document progress, and communicate with patients and families about injury, progress and potential outcomes. This position emphasizes high-quality, patient-centered care in a dynamic team setting.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
