Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients, develop and adjust plans of care, and collaborate with facility staff to achieve measurable functional goals.

Qualifications

• Licensed Occupational Therapist (OT) in good standing.

Responsibilities

• Evaluate and treat patients in a SNF setting following an individualized plan of care.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants (OTAs).
• Collaborate with facility staff to ensure high-quality patient care.
• Develop plans of care and set measurable functional goals.
• Educate patients and family members about injuries, progress and outcomes.

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
