Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients, develop individualized plans of care, and set measurable functional goals to support patient progress.

Responsibilities include documenting evaluations, re-evaluations, discharges, and daily progress notes, as well as supervising Occupational Therapy Assistants and collaborating with facility staff to ensure outstanding patient care.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients and educating them on injury, progress and probable outcomes
• Re-evaluating patient progress and revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
• Supervising Occupational Therapy Assistants
• Collaborating with facility staff to ensure quality patient care

• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
