Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our dynamic team in a SNF setting. You will evaluate and treat patients following an individualized plan of care.

You will collaborate with facility staff to ensure excellent care, develop plans of care, set measurable goals, and educate patients and families about progress and outcomes. You will supervise therapy assistants and ensure timely documentation of evaluations and progress notes.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients and addressing inquiries about injuries, progress and likely outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
• Supervising Occupational Therapy Assistants and coordinating care with facility staff

• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
