Job summary

Fox Hollow Post Acute is seeking a Staff Occupational Therapist to evaluate and treat patients, communicate with families, physicians, and the health team, and maintain documentation in medical records per state practice Act.

You will supervise Certified Occupational Therapy Assistants, participate in care conferences, lead in-services, and contribute to discharge planning and caregiver training to promote patient independence.

Qualifications

• Bachelor's degree in Occupational Therapy; Masters/Doctorate preferred.
• Licensed Occupational Therapist in the state.
• Proficient with Casamba and Point Click Care.
• Ability to communicate with managers, coworkers and families.
• Continuing education requirements maintained.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop and obtain approval for treatment plans.
• Treat patients per physician treatment plan.
• Supervise Certified Occupational Therapy Assistants and aides.
• Record progress notes and documentation per state regs.
• Lead in-services and staff training on therapy processes.
• Participate in Patient Care Conferences and Rehabilitation Meetings.
