Job summary

Homestead Post Acute is seeking a Staff Occupational Therapist to evaluate and treat patients, communicate with families and physicians, and maintain documentation in medical records. The role includes supervision of COTA and aides, and collaboration with the health team to plan and monitor patient care.

The clinician will conduct staff trainings, participate in care conferences, and ensure compliance with state practice acts and payer requirements. Licensure and ongoing CE are required.

Qualifications

• Bachelor's degree in Occupational Therapy; prefer graduates with a Master’s or Doctorate.
• Licensed as an Occupational Therapist in the state; active license required.
• Proficient in Casamba and Point Click Care computer systems.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of physician referral.
• Develop treatment plans and obtain physician approval.
• Treat patients per the physician plan.
• Supervise COTA and aides in direct patient care.
• Communicate patient progress with supervisor and health team.
• Participate in patient care and rehabilitation conferences as needed.
• Record evaluations, daily notes, weekly progress notes, and recertifications.
• Instruct families and nursing staff for discharge preparation.
• Recommend necessary durable medical equipment to aid mobility.
• Report equipment problems for maintenance.

Education

Bachelor's degree in Occupational Therapy

Licensed as an Occupational Therapist in the state

Masters or Doctorate in Occupational Therapy
