Job summary

Sage Creek Post-Acute in is seeking a Staff Occupational Therapist to evaluate and treat patients, develop treatment plans, and document services in medical records. You will train staff, supervise COTA and aides, and coordinate with physicians, families, and the health team.

Ideal candidates hold a state-licensed OT degree (Bachelor's required; Masters/ Doctorate preferred), maintain continuing education, and communicate effectively.

Qualifications

• Bachelor's degree in Occupational Therapy; state license to practice.
• Masters or Doctorate in Occupational Therapy preferred.
• Proficient computer skills including Casamba and Point Click Care.
• Ability to read technical procedures and policy manuals.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral; document delays if needed.
• Develop treatment plans and obtain physician approval.
• Treat patients per physician plan and document progress.
• Supervise COTA and aides in patient care; coordinate with supervisor and team.
• Communicate patient progress to families and health team; participate in care conferences.
