Responsibilities

• Evaluate residents to determine OT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise COTA and aides in resident care per state practice act.
• Communicate progress and discharge planning with physicians, staff, residents, and families.
• Maintain timely, compliant documentation in medical records.
• Participate in facility meetings and in-services as needed.
• Educate residents and caregivers on home exercise and safety techniques.
• Support with equipment ordering for independence.
