Job summary

Beachwood Post Acute & Rehab is seeking a Staff Occupational Therapist to evaluate residents and develop individualized treatment plans to enhance mobility and independence. The role requires supervising OT aides and coordinating with physicians, families, and the care team.

You will document care accurately, educate patients and families, and participate in care conferences to ensure safe, effective rehabilitation and discharge planning.

Qualifications

• Bachelor’s degree in Occupational Therapy; Master/Doctorate preferred.
• Proficient with Net Health and Point Click Care systems.
• Ability to read and comprehend policy and procedure manuals.
• Ability to present information to managers, coworkers and families.

Responsibilities

• Evaluate residents to determine OT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise Occupational Therapy Assistants and aides in direct resident care.
• Communicate progress, goals and discharge planning with physicians and families.
• Participate in Resident Care and Rehabilitation meetings as needed.
• Provide in-services on training programs for staff as needed.
• Complete timely, accurate documentation per facility and regulatory requirements.
• Educate residents and families on goals, home exercise, safety techniques and fall prevention.
