Bridge Crest Post Acute is seeking a dedicated Occupational Therapist to join our care team in a hands-on clinical role. You will evaluate residents, develop individualized treatment plans, and supervise COTA staff to support functional independence.

You will collaborate with physicians and nursing staff, document thoroughly, and educate residents and families on treatment goals and home programs. This is an on-site position with a patient-centered, team-driven environment.

Qualifikationen

• Bachelor's degree in Occupational Therapy; Masters/Doctorate preferred.
• Proficient in Net Health and Point Click Care data input.

Aufgaben

• Evaluate residents to determine OT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise COTA and OT aides in resident care per state practice acts.
• Communicate with physicians, nursing staff, residents, and families about progress and discharge planning.
• Complete timely, accurate documentation in accordance with regulations.
• Educate residents and families on treatment goals and home exercise programs.
• Arrange and facilitate necessary durable medical equipment for independence in ADLs.

Bachelor's degree in Occupational Therapy

Masters or Doctorate in Occupational Therapy (preferred)
