Job summary

Fox Hollow Post Acute is seeking a Part-time Physical Therapist to evaluate patients within 24 hours of referral and develop treatment plans with physician approval. The role includes supervising PT assistants and coordinating care with nursing staff, while participating in discharge planning and family education.

The candidate will document evaluations and progress notes, and contribute to patient conferences and staff trainings.

Qualifications

• Bachelor's degree in Physical Therapy; Master/Doctorate preferred.
• Proficient with Casamba and Point Click Care.
• Ability to read and interpret technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Strong verbal and written communication with managers, coworkers and families.
• Apply fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems and adapt to changing priorities.
• Follow written, oral or diagrammatic instructions.

Responsibilities

• Evaluate patients within 24 hours or 1 business day after referral.
• Develop treatment plan and obtain approval from referring physician.
• Treat patients per physician plan and supervise PT assistants.
• Communicate patient progress in conferences and with health team as needed.
• Record evaluations and progress notes per state requirements.
• Instruct families or nursing staff in maintenance and caregiver training for discharge.
• Participate in discharge planning and equipment recommendations to facilitate mobility.
• Coordinate with supervisor regarding patient progress.
