Crystal Creek Post-Acute in Stockton, CA seeks a licensed Occupational Therapist (OT) for a full‑time role. You will evaluate and treat patients, develop plans, educate families, and document care per facility guidelines, collaborating with the care team to optimize outcomes.

The ideal candidate holds an active California OT license, current BLS/CPR, and experience in long‑term care. We offer a sign‑on bonus, relocation assistance, and a comprehensive benefits package for full‑time staff.

Formación

• Graduate of an approved curriculum in Occupational Therapy.
• Active license as Occupational Therapist and able to practice in California.
• Active BLS/CPR
• In good standing with regulatory agencies and licensing boards.
• Working knowledge of Medicare and other payor sources.
• Full knowledge of resident's rights.
• Experience in long term care preferred
• Able to work on multiple tasks at the same time.

Responsabilidades

• Provides and directs all aspects of quality patient care including evaluation and treatment.
• Develops treatment plans and provides treatment aligning with reimbursement levels.
• Screens individuals who may benefit from therapeutic intervention.
• Completes documentation including evaluations and progress notes.
• Ensures safety of self, patients and others at all times.
• Collaborates with interdisciplinary team to achieve quality patient care.
