Job summary

South Creek Post Acute is seeking a Staff Occupational Therapist to evaluate and treat residents, communicate with families and physicians, and document services in medical records. You will develop individualized plans to improve mobility and independence and supervise support staff, ensuring compliance with state practice acts.

The role requires a Bachelor's in Occupational Therapy (Masters/Doctorate preferred), licensure in the state, and strong communication with an interdisciplinary team.

Qualifications

• Bachelor’s degree in Occupational Therapy is required; Masters/Doctorate preferred.
• Experience supervising COTA, aides, and students is expected.
• Proficient in documentation and computer systems used for health records.

Responsibilities

• Evaluate residents to determine OT needs and rehab potential.
• Develop and implement individualized treatment plans to improve mobility and independence.
• Supervise Certified Occupational Therapy Assistants and aides.
• Maintain timely, accurate documentation in medical records.
• Educate residents, families, and caregivers on goals and home programs.
• Communicate with physicians, nursing staff, and families about progress and discharge planning.
