Job summary

South Creek Post Acute is seeking a PRN Occupational Therapist to evaluate and treat residents, communicate with families and physicians, and maintain complete documentation. The OT will supervise COTAs, assist with staff training, and contribute to discharge planning and quality improvement initiatives.

The role requires a Bachelor's in OT, state licensure, and strong documentation and communication skills.

Qualifications

• Bachelor's degree in Occupational Therapy; graduates of Masters or Doctorate preferred.
• Proficient in computer skills such as inputting data into Net Health and Point Click Care.
• Ability to read technical procedures and policy manuals; present information clearly.
• Ability to apply fractions, percentages, ratios and proportions to practical situations.
• Licensed as an Occupational Therapist in the state; CPR certification if required.

Responsibilities

• Evaluate residents to determine OT needs and rehab potential per physician orders.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise Occupational Therapy Assistants and staff; coordinate with health team.
• Communicate progress with physicians, nursing staff, residents and families.
• Complete timely documentation and maintain a safe treatment environment.
• Educate residents and caregivers on goals, home exercises, safety, and fall prevention.
