Job summary

Crane nursing and rehab is seeking a compassionate Occupational Therapist (OT) to join our in-house team on a PRN basis. You will evaluate residents, develop treatment plans, and promote independence in daily activities.

The ideal candidate holds a valid Texas OT license, has experience in SNF or long-term care, and can collaborate with an interdisciplinary team. We welcome new graduates and offer flexible schedules.

Qualifications

• Active and valid Texas Occupational Therapy license.

Responsibilities

• Consult with patients to evaluate physical condition and its impact on daily life.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADL/IADL performance.
• Assess home environments and recommend modifications.
• Recommend adaptive equipment and training for independence and safety.
• Ensure treatments follow the plan of care.
• Monitor patient response and adjust interventions with the interdisciplinary team.
• Provide education and support to patients, family, and caregivers.
• Complete documentation in compliance with company policies and state/federal regulations.
• Attend required meetings designated by the Director of Rehab.
