Job summary

Sienna Nursing & Rehabilitation is seeking a compassionate Occupational Therapist (OT) for a PRN position to support our in-house team. You will focus on improving residents' independence and safety through meaningful activities and functional outcomes in a SNF setting.

The role requires an active Texas OT license and collaboration with an interdisciplinary team. Experience in SNF or long-term care is preferred, and familiarity with Net Health documentation is a plus.

Qualifications

• Active Texas Occupational Therapy license.
• Licensed OT in good standing with regulatory boards.
• SNF/long-term care experience preferred.
• Knowledge of OT assessment and treatment principles.

Responsibilities

• Evaluate patient condition and its impact on ADLs and IADLs.
• Develop individualized treatment plans to maximize independence.
• Recommend adaptive equipment and home modifications.
• Complete documentation in a timely and accurate manner.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Provide patient and caregiver education and support.
