Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients, develop plans of care with measurable goals, and collaborate with facility staff to ensure high-quality care.

You will re-evaluate progress, adjust plans, and provide education to patients and families while supervising Occupational Therapy Assistants. Timely documentation is required to support patient progress and outcomes.

• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, addressing their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
