Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our SNF team in Illinois. You will evaluate patients, develop plans of care, set measurable functional goals, and treat injuries while communicating progress to patients and families.

In this role you will collaborate with facility staff, re-evaluate progress, maintain timely documentation, and supervise Occupational Therapy Assistants to ensure high-quality patient care and outcomes.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
• Supervising Occupational Therapy Assistants
