Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients, develop individualized plans of care, and collaborate with facility staff to ensure optimal patient outcomes.

The role includes documenting evaluations, re-evaluations, discharges, and daily progress notes, as well as supervising Occupational Therapy Assistants. You will educate patients on injury, progress, and expected outcomes.

Responsibilities

• Evaluate new patients, develop plans of care and set measurable, functional goals.
• Treat patients and educate them on injuries, progress and outcomes.
• Re-evaluate patient progress and adjust plans of care as needed.
• Complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants and collaborate with facility staff.

• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
