Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients, develop individualized plans of care, and set measurable, functional goals based on patient needs.

Additionally, you will educate patients and families about injuries and progress, re-evaluate progress, revise plans of care, and ensure timely documentation. You will supervise Occupational Therapy Assistants to maintain high-quality care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
