Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our dynamic team in a skilled nursing facility setting. You will evaluate and treat patients, develop plans of care, and set functional goals tailored to each patient’s needs.

You will educate patients and families about injuries, progress, and likely outcomes, re-evaluate progress, and adjust plans as needed. Supervising OTAs and documenting care will be integral to daily responsibilities.

Qualifications

• Active Illinois OT license or eligibility.
• Experience in skilled nursing facilities preferred.
• Strong communication and documentation skills.
• Ability to set measurable, functional goals for patients.

Responsibilities

• Evaluate and treat patients in a SNF setting.
• Develop plans of care and set measurable goals.
• Provide patient and family education on injuries, progress and expected outcomes.
• Supervise Occupational Therapy Assistants.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
