Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapists to evaluate and treat patients in a skilled nursing facility (SNF) setting within an individualized plan of care. Responsibilities include patient evaluation, goal setting, education, and collaboration with facility staff.

You will re-evaluate progress, adjust plans, and ensure timely documentation while supervising Occupational Therapy Assistants to deliver high quality care.

Qualifications

• OTs are required to hold a current OT license.
• Experience in SNF settings is preferred.
• Strong documentation and patient-care skills are essential.

Responsibilities

• Evaluate and treat patients in a SNF setting.
• Develop plans of care and set measurable goals.
• Provide education to patients and families on injuries and progress.
• Re-evaluate patient progress and revise plans as needed.
• Complete timely documentation including evaluations and progress notes.

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
