Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to join our growing team in a SNF setting in Illinois. You will evaluate and treat patients, develop plans of care, and set measurable functional goals.

You will also document evaluations and progress, collaborate with facility staff, and supervise Occupational Therapy Assistants to ensure high-quality patient care and outcomes. This role emphasizes patient education, discharge planning, and multidisciplinary communication to support

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
