Enhance Rehabilitation is actively seeking an Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients, develop individualized plans of care, and collaborate with facility staff to ensure high-quality patient outcomes.

You will re-evaluate progress, adjust goals as needed, and document evaluations, re-evaluations, discharges and daily progress notes. Supervising OTAs is also required to maintain care standards.

Formación

• Qualified Occupational Therapist with valid state license.
• Experience in SNF settings preferred.
• Strong patient evaluation and documentation skills.

Responsabilidades

• Evaluate and treat patients in SNF following individualized plans of care.
• Develop plans of care and measurable functional goals.
• Re-evaluate progress and revise plans of care as needed.
• Complete timely documentation including evaluations, re-evaluations, discharges and progress notes.
• Supervise Occupational Therapy Assistants and collaborate with facility staff.

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
