Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our dynamic team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

You will collaborate with facility staff, complete timely documentation, educate patients and families, and supervise Occupational Therapy Assistants to ensure high-quality, consistent care. Strong communication and compassion are essential to helping patients regain independence.

Qualifications

• OT degree from an accredited program.
• Current state OT license.
• SNF or rehab setting experience is preferred.

Responsibilities

• Provide excellent care for patients.
• Collaborate with facility staff to ensure excellent care.
• Evaluate new patients and develop plans of care.
• Re-evaluate progress and adjust goals.
• Complete evaluations, re-evaluations, discharges, and daily progress notes.
• Supervise Occupational Therapy Assistants.

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
