Job summary

Enhance Rehabilitation in Illinois is seeking a dedicated Occupational Therapist to join our SNF team. You will evaluate and treat residents, develop plans of care, and work with staff to achieve measurable functional goals.

You will supervise Occupational Therapy Assistants, complete documentation, and educate patients and families about progress and outcomes. This role offers a collaborative environment and opportunities to impact quality of life.

Qualifications

• OT licensure in good standing
• Experience in SNF settings preferred
• Ability to develop functional goals for residents
• Excellent communication with patients, families, and staff
• Knowledge of documentation and regulatory requirements

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
