Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our team in a SNF setting. You will evaluate and treat patients, develop plans of care, and set functional goals within an individualized plan of care.

The role includes supervising Occupational Therapy Assistants, collaborating with facility staff, and maintaining timely documentation of evaluations, progress notes, and discharges.

Qualifications

• Licensed Occupational Therapist in good standing
• Experience evaluating patients and developing plans of care in SNF or similar setting
• Ability to set measurable, functional goals and track progress

Responsibilities

• Evaluate new patients and develop plans of care with measurable goals
• Treat patients and educate patients and families about injuries, progress and outcomes
• Re-evaluate patient progress, revise plans of care, and reset goals
• Complete timely documentation: evaluations, re-evaluations, discharges and daily progress notes
• Supervise Occupational Therapy Assistants and collaborate with facility staff
