Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our dynamic team in a SNF setting. You will evaluate and treat patients, develop individualized plans of care, and collaborate with facility staff to ensure excellent patient outcomes.

You will document evaluations, re-evaluations, discharges and daily progress notes; supervise Occupational Therapy Assistants; and educate patients and families about injury, progress, and outcomes.

Qualifications

• Licensed Occupational Therapist (OT) in Illinois.
• Experience evaluating and treating patients in SNF settings is preferred.
• Ability to supervise Occupational Therapy Assistants and collaborate with facility staff.

Responsibilities

• Evaluate and treat patients in a SNF setting following an individualized plan of care.
• Develop plans of care and set measurable, functional goals.
• Complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants and educate patients and families.
