Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our SNF care team. You will evaluate and treat patients in a skilled nursing facility setting, following an individualized plan of care and contributing to optimal functional outcomes.

Responsibilities include evaluating new patients, developing plans of care with measurable goals, treating patients, educating them about injuries and progress, re-evaluating progress, revising plans, and completing timely documentation.

• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
• Supervising Occupational Therapy Assistants
• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborating with facility staff to ensure excellent care for you patients
