Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our dynamic team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, while collaborating with facility staff to maintain excellent patient outcomes.

You will also document evaluations, progress notes, and discharges, and supervise Occupational Therapy Assistants to uphold high standards of care and customer service for patients and families.

Qualifications

• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
• Collaborating with facility staff to ensure excellent patient care.
• Providing excellent care and customer service to patients and family members.

Responsibilities

• Evaluate and treat patients in a SNF setting.

• Re-evaluate patient progress and revise plans as needed.
