Job summary

Elara Caring in Louisiana is seeking a Licensed Vocational Nurse to provide skilled home health care to patients in their homes. You will follow care plans, monitor progress, and collaborate with the RN Case Manager to update plans as patient needs change.

This role requires an LVN/LPN license, nursing diploma, and at least 1 year of clinical experience; valid driver’s license or reliable transport is essential for travel between patient homes.

Qualifications

• Current State License as LVN/LPN in Louisiana.
• Graduate of an accredited nursing program (Certificate, Diploma, or Associate).
• Minimum 1 year of clinical care experience.

Job Description:

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
