Job summary

Elara Caring is hiring a Licensed Vocational Nurse (LVN) in Topeka, KS to support home health patients. You will follow individualized care plans, monitor progress, and educate patients and families on self-care techniques, working with an RN Case Manager to adjust plans as needed.

The role requires 1 year of clinical care experience, an active Kansas LVN license, and the ability to lift up to 50 pounds. Travel between patient homes with reliable transportation is expected.

Qualifications

• Current LVN license in Kansas.
• Graduate of an accredited nursing program.
• 1 year of clinical care experience.
• Ability to lift up to 50 pounds and assist patient transfers.
• Valid driver's license or reliable transportation for travel between homes.
• Home health or hospice experience preferred.

Responsibilities

Job Description:

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.

• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
