Job summary

Elara Caring seeks a Licensed Vocational Nurse to join our home health team in Centerville, IA. You will follow individual care plans, administer medications, and coordinate with RNs to adjust care as patient needs evolve.

The role emphasizes compassionate patient care, caregiver education, and proactive management to improve outcomes in a home health environment. Prior hospice/home health experience is preferred.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Experience in a hospice or home health environment is preferred.
