Job summary

Elara Caring is hiring a Licensed Vocational Nurse to provide in-home care across patient plans, working Sat/Sun 12-hour shifts plus one weekday. You will monitor patients, document med regimens, and coordinate with the care team to maintain quality outcomes.

Must have an LVN license in Ohio, 1 year clinical experience, and reliable transportation. Hospice/home health experience preferred; benefits include insurance, 401(k), tuition support, PTO, and mileage assistance.

Qualifications

• Active LVN license in Ohio.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Must possess a valid driver's license or have reliable transportation to travel between patient homes and locations.
• Experience in hospice or home health preferred

Job Description:

SCHEDULE- SAT/SUN & 1 DAY DURING THE WEEK 12 HR SHIFTS

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
