Job summary

Elara Caring in Baton Rouge, LA is seeking an experienced Licensed Vocational Nurse (LVN) to follow patient care plans and physician orders across home health visits. You will monitor progress, educate patients and caregivers, and work with the RN Case Manager to adjust care plans as patient needs change.

The role requires at least 1 year in a clinical setting, the ability to lift up to 50 pounds, and a valid driver's license or reliable transportation.

Qualifications

• Current State LVN/LPN license.
• Graduate of an accredited certificate, diploma, or associate nursing program.
• Minimum 1 year of experience in a clinical care setting.
• Ability to lift/push/pull up to 50 pounds regularly.
• Valid driver's license or reliable transportation for travel to patient homes.
• Hospice or home health experience is preferred.

Responsibilities

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
