Job summary

Elara Caring in Bloomington, IN is seeking a Licensed Practical Nurse to join our home health team. You will follow individualized care plans, administer medications per orders, and support patients and families in managing health at home.

This role collaborates with the RN Case Manager, requires a valid LPN license, and at least one year of clinical experience. A driving record and reliable transportation are essential to travel to patient homes.

Qualifications

• Current State License as a Licensed Practical Nurse (LPN).
• Graduate of an accredited nursing program (Certificate, Diploma, or Associate).
• 1 year of experience in a clinical care setting.
• Valid driver’s license or reliable transportation to travel to patient homes.
• Experience in hospice or home health preferred.

Responsibilities

• Follow each patient’s individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Help patients and caregivers learn self-care techniques to support health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and ER visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
