Job summary

Baptist Memorial Hospice, part of the Optum family, is seeking PRN Registered Nurses to deliver comprehensive hospice care after regular hours. Responsibilities include phone triage, after-hours visits, and initial evaluations within 48 hours of referral.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.

Responsibilities

• Triage patient needs by phone until an in-person visit by a nurse.
• Respond to crisis calls after hours with urgency.
• Perform initial nursing evaluation within 48 hours of referral.
• Conduct afterhours visits including admissions and recertifications.
• Assess and document patient/family physical, psychosocial, and environmental needs.
• Document problems, goals, interventions, and outcomes.
